Hi there, I don’t have any Windows 11 domain joined PC to test the extension behavior.
Have you checked the focus options?
Have you been running it using SYSTEM account or normal user account?
In fact, we have computers connected to a domain and others to Intune. We are going to move everyone to Intune soon. I am testing with my computer connected to the domain. This morning I tested with an Intune computer in system context and I didn’t see anything appear on the screen. Here is the log of my installation. Maybe it will help to understand.

My god, the log is impossible to read, check how to post it the right way.
But… DeployMode [NonInteractive] means no Toast Notification nor windows will appear.
I did some more tests. I modified my command line in my Azure console to be in interactive mode. But it doesn’t work. During the initialization phase, it indicates “Session 0 detected” and then it falls into non-interactive mode. Should toast notifications work in Intune?
The problem with Intune is that the process is executed in NonInteractive mode, that’s the limitation, you can use ServiceUI.exe to make it Interactive.
I have some ideas about how to make the Intune process interactive automatically, ideas but no time 
By default you won’t see any Toast Notification nor dialogs nor prompts in Intune, even the Toast Notification extension is present or not.

Is there a way to stack the ‘Blocked Execution’ notification on top of the ‘Installation Progress’ notification? I find at the moment that triggering the ‘Block’ notification causes the ‘Progress’ notification to close and once the ‘Block’ notification closes (based on a timer?) there is then no longer any visual clue to the continuing installation. If it could be stacked, then when the ‘Blocked’ one closes, we would still have the ‘Installation’ one in place.

The short answer is NOT.
Regular Toast Notifications can be displayed with short (7s) or long (25s) duration if no scenario is given.
The interactive Toas Notifications, that have action buttons, are persistent and the scenario is alarm.
To mantain the Installation Progress visible after the long/short duration I keep it updated in background. Causing the Block Execution one to never appears.
(Not to mention that both of the above processes are asynchronous).
So, in order to show the Block Execution i have to sacrifice the Installation Progress.
If there any way to reshow the closed Installation Progress? Maybe yes, but would request some complex coding and don’t know if have enough time.

I am just testing it out with VLC player and I am testing out the defer button however, it gives me the error listed above and brings back the original prompt. I switch fallback to the origina prompt off in the XML however, I still get an error.
This error also appears if I click continue instead of defer as well.
Here is what I tried:
Tried in both 3.84 and 3.92 PSADT versions
Ran as regular, admin, and system context.
Tried a different exe/msi
Any advice?

The feature you mention in your post is not implemented, by default if any Toast Notification reaches the end of the function without a result the original function is called, you can’t switch this off (without editing the ps1 file).
I need a complete debug log to see what is going on…
If I understood it well, the Toast Notification is shown but when you click any button (Defer, Continue) the original prompt is displayed?

Am really struggling with the intune deployment side of it.
Executing deploy-application.exe with an elevated command prompt the TOAST notifications work perfectly, which is expected, thank you.
The problem is, when I try and deploy via Intune, it fails everytime.
Using PSexec to run CMD within the system environment i receive the following errors in my log and the Toast notifications nor does the app install.
Firstly, I receive exit code 1 from psexec. Checking the PSADT log files I can see the following errors:

With Intune you need to set the deployment to run as System and this would be the install command I would use (N.B. I include the ServiceUI.exe in the root of my Intunewin file):
ServiceUI.exe -Process:explorer.exe Deploy-Application.exe -DeploymentType "Install"

P.S. You can only set it to run as System (or User) when you first build the deployment, if it is set wrong, you need to recreate the deployment

I have no longer access to a SCCM / Intune environment, it was a solution implemented for the last job I was assigned.
I’m afraid I can’t help you right now. 
Even though the error is coming from the RunAsActiveUser extension when trying to impersonate, the parent process cannot delegate, look for ‘SeDelegateSessionUserImpersonatePrivilege’ in the web.
Any update on this issue? I’m having the exact same problem and would really like to use this

Hi there, do not use PSEXEC, just create a new application whitin SCCM and try it, enable debug logging and paste the result just to see where it fails.

I’ve uploaded my logs but was hidden to be reviewed. The deployment is from Intune not SCCM. I actually think it might be more related to the device being enrolled in Azure AD as it fails using psexec too while on a standard device with a local account it works perfectly.
I’ve seen this in the previous post. Apparently Intune creates the process without the ‘SeDelegateSessionUserImpersonatePrivilege’ token needed to impersonate the user and show the Toast Notification when deployed as System.
Intune is a blackbox don’t know if I could fix this without the proper testing environment.
You could try commenting 524-537 lines included and 579 in file /AppDeployToolkit/PSADT.RunAsActiveUser/RunAsActiveUserExtension.ps1
